Collagen has long served as an osteoconductive bone scaffold, yet outcomes remain inconsistent. We functionalize nanostructured collagen with exosomes from adipose-derived stem cells (ADSCs), an abundant, scalable source, turning a passive scaffold into an active, signal-delivering matrix. Exosomal collagen-binding integrins (e.g., α2β1) anchor the vesicles and activate FAK, RhoA–ROCK and MEK–ERK1/2, upregulating RUNX2 and osteogenic genes (OCN, ALP, BSP). The exosomes also deliver miR-375, miR-21, miR-29, miR-126, BMP-2, VEGF and TGF-β1, coupling angiogenesis with osteogenesis. This cell-free, low-immunogenicity dual-cue platform suits bone-defect filling and craniofacial reconstruction.
